Attachment Report
Day 1
We were brought to this simulation lab at NUH. It was my first time getting a hands-on experience to deal with simulated real-life situation. I did not realize that the ‘patient’ can actually talk and respond to us. When it was my friend’s turn to do so, she wondered why she did not hear any heartbeat. It was then that we found out that the patient was dead and we had killed him.
It was quite fun though. The patient will respond to us each time. Of course, all these were simulated based on the situation each time. For example, when the patient was having breathing difficulty and so many of us crowded around him with one person trying to listen to his heartbeat, he suddenly said ” No no, you don’t know what you are doing?” Quite funny though. As for life-threatening situation, we would sometimes hear, “ I want my wife, call my wife.”
We also learnt that the doctors do not shock the patient when you hear the sound and the line goes flat on the screen. They do that when the amplitude gets smaller and smaller, close to being straight. Besides, the screen also shows the blood pressure and carbon dioxide percentage.
Day 2
On the second day, I was attached to the geriatrics department. In the morning, we went around visiting the wards. I also get to see the doctor inserting a plug into the vein of the patient as they had to inject the antibiotics in. This seems a difficult job as the elderly usually have fragile veins which burst very easily. Thus, they usually get bruises within hours and they have to change the plug regularly. Sometimes there will be bumps due to the resistance. I was told that if you feel any resistance, give in to that, do not try to poke in any further. (That is because he had tried it before and the patient really screamed in pain). Before you insert the plug, you have to tie a knot using glove or a band, this is to stop the blood from flowing back to the heart as we are inserting it into the veins. Then, he will beat up the veins so that they will appear fatter and greener so that it is easier to locate it while poking it in.
Lynette and I were attached to the same ward. I had to say that Flora’s very nice to nice! Flora n ming yao are both final yr students who were currently doing the attachment n get to practise in order to prepare for them to be a HO next yr. During lunch, they kept offering to treat us drinks. I finaly realized what life is like being a doctor, coz flora kept saying we shall go for lunch soon..n soon..n soon..from 11 to 1230(which she targeted the time for her to complete her report) den theres an emergency..so we had lunch at 2plus instead..by tt time, we were all past the hunger mood..u hav no time to eat if u wanna be a doc! No wonder they are all so healthily thin cos they dun snack when they are hungry too, n 1 MO even skip(or shall I say miss?) her breakfast n her last meal was dinner e prev day coz shes on call e whole nite..imagine! 30 plus h of work! If ur lucky..u go home at 2 e next day, if not, lyk ming yao, he talked to us n went home at 6? Coz we left at ard 6 n hes stil there..
Day 3
I was attached to the A&E department. I had to change out of my casual wear to the blue shirt which had the words A&E on it. I have seen some real-life situation such as the stitching of a finger, removal of fingernail, putting the patient to sleep while the doctor performs some minor surgery.
Oh man! Itz reli gross when seeing the doc poke n poke to remove the fingernail..coz the blood reli splash out..haiyo.
“Doing medicine is clearly not for money. You must have the passion and you must be one who love to communicate with people as you will need to interact with the occupational therapist, pharmacist, all those in medical field, the patients’ families, and most importantly, you must be one who understand the patients’ needs and feelings. We are human after all.” This is from one of the HO that we had met.
The HO and MO were lyk “ oh! u doing attachment isit? Dun do medicine! Dun do medicine!” coz the HO had to do on-call duties..abt 8 a month e most. N final yr students had to do it every 4-5 days. They were sick of dis life.
hmm..my hol has juz began..coz im doing nyaa stuff everyday..cip..yay! completed 41 h of cip during the hol! n my nyaa residential proj is done too!
